Specimen Type and Gross Description Mean More Than You Think
The first section lists what the pathologist received. A needle biopsy gives less tissue than an excisional biopsy. A lumpectomy specimen differs from a mastectomy specimen. The size and weight of the specimen matter. I worked a breast cancer case where the defendant claimed the tumor was small and early stage. The gross description listed a 4.2 cm mass with skin involvement. The defense expert had based his opinion on radiology alone. We used the pathology report to destroy his testimony. The gross description also notes if the specimen arrived fresh, in formalin, or fragmented. Fragmentation limits what the pathologist sees and introduces reasonable doubt you need to address.
Microscopic Description Reveals Tumor Behavior
This section describes what the pathologist sees under the microscope. The histologic type tells you what kind of cancer you have. Adenocarcinoma, squamous cell carcinoma, and small cell carcinoma all behave differently. The tumor grade measures how abnormal the cells look. Grade 1 means well-differentiated and slow-growing. Grade 3 means poorly differentiated and aggressive. I reviewed a colon cancer case where the defendant claimed a six-month delay caused no harm. The pathology showed grade 3 adenocarcinoma with lymphovascular invasion. High-grade tumors metastasize fast. That six months mattered. Stage 3 and Stage 4 tumors do not. I worked a lung cancer case where the plaintiff was diagnosed at Stage 4. We obtained records showing a lung nodule on CT scan 18 months earlier. I prefer synoptic reports because nothing gets buried in paragraph form. You see every critical detail at a glance. Some older reports use narrative format. You must read every word.
